Step 4-Select Authorization Form


Find the Right Form for You

  • Once you select the "Next" button, the "Select Authorization Form" modal will appear. 
  • SamaCare offers a variety of forms to support different Prior Authorization types. To ensure you're using the most appropriate one, simply locate and select the form that best matches your specific authorization needs. For your convenience, forms are listed based on helpful criteria such as:
    • Recommended - We've prioritized any forms that are likely a good fit for your current request based on the insurance information you provided.
    • Recently Used - We've included the recently used forms for this request type.
    • Favorites - We've included any previously favorited forms related to this request type.

Understanding the Current View 

  • The Heart Icon - Select this icon to favorite your Form.
  • "Use this form" - After selecting the form of your choice, select "Use this form".
  • "Form Missing" - If the form you would like to use is missing, select "Form Missing".
  • "Cancel" - If you need to edit the insurance information you previously entered, select "Cancel".
    • The "Zoom" Feature - Select this option to view the full version of any form and easily navigate through populated form.
