Log in to SamaCare and navigate to your authorizations view. 

Select "Auth"-->"Form" 

• This is located in the top right corner of your screen.

• To the right of your screen, you will see the progress tracker of your submission.
  • Pre Submssion
    • Initiate Prior Authorization
    • Request & Patient Details
    • Provider & Location Details
    • Form Details
    • Review Sign & Submit
  • Authorization Submitted
  • Response Received
    • Insurance Response
