You will need certain information to begin submitting your portal authorization. Below are the details you will need in order to fill out and submit your prior authorization successfully. Be sure to have the member ID card in hand!
Payer Portal Information
- Many insurance payers may use different portals for submissions of prior authorizations depending on type of authorization being submitted. Make sure you know which portal you are wishing to submit to and ensure that this is one of our supported portals with SamaCare. You can locate a list of supported portals on our Portal Integrations FAQs.
- If your payer portal is not supported, you can request a missing portal by following the steps outlined here.
- Until we can integrate the requested portal, you will need to submit via form. You can find steps here on Submitting New Form Authorizations.
Patient Demographics
- First and Last Name of the patient.
- Patient date of birth.
- Height & Weight
- Gender
- Patient Address
- Patient Phone Number

Drug Code(s)
- HCPCS
- Quantity
Patient Insurance Information
- Insurance Type
- Medicare
- Medicaid
- Commercial
- Other
- Member ID
- Issuing State
- Insurance Company
You can locate all of the patient information needed for this section just by having the patients member ID card in hand!
Start Date of Service
- The date that you wish to have the service starting on to provide to the insurance payer for review.
ICD-10 Code(s)
- All diagnosis codes related to your prior authorization request.
Provider Details
- First and Last Name
- Specialty
- NPI
