In this step you will be entering provider and location details for your prior authorization.  

Requester Details

• Your name and phone number will populate here. This can be edited if a change is required.

Provider Details 

• Provider details will be entered in one of two ways:
  • Entered manually.
  • Pre-populated and selected.

If the provider you are looking for does not populate, the provider will need to be entered manually.  You can use the "Create + Set New" option to save this provider for future use.  If your referring provider is different from your servicing provider, utilize the tab "Referring Provider" to add this information here.

See steps on how to edit an existing provider here. **Please note that only a user with administrative capabilities can edit provider information.

Location Details

• Location details will be entered in one of two ways:
  • Entered manually.
  • Pre-populated and selected.

If the location you are looking for does not populate, the location will need to be entered manually. 
You can utilize the "Create + Set New" option to save this location for future use.
